Systems, Inc. initially developed a patient
centric medical home application to improve
the quality of healthcare in Vermont.
Our premise was to offer powerful tools to
patients that would allow them to become more
active in the treatment of their own health.
It is also intended to enhance the
communication and relationships between
patients and their health care providers.
Our medical
home software has the capability to "attach"
to other systems, such as electronic medical
records (EMRs), as well as our other
healthcare programs like our
Chronic Disease Registry
and Closed Social
Network applications. The software
can also connect with local hospitals,
visiting nurses, and Health Information
Exchange (HIE) structures.
The medical
home software then evolved to include
additional levels of data collection,
reporting, and communication.
Provider-level features were added to allow
patients to choose what providers they wish
to correspond with, and what patient-level
data those providers can or cannot see.
Any type of provider can use the system,
whether it's a single doctor, a large
practice, visiting nurses, or a hospital
physician. Providers can use the
built-in secure messaging system, and
administer ad hoc surveys to their patients.
Organizational-level features were also added
to the medical home application.
Organizations can maintain the providers who
would have access to the system. Like
providers, organizational-level personnel can
use the secure messaging system to
communicate with their providers, and
administer surveys. Finally,
organizational-level personnel can view
activity for their providers and their
patients. They can also upload data to
the system, including patient demographics,
vitals information, and lab test results.
